Camp Emergency Information

Greetings to Parents of Campers (any child enrolled in a 4- or 5-day per week Program (i.e. Camp, JYSC, YSC, etc.).

Summer is here and we have many exciting activities planned for your campers! In our continuing quest to provide excellent customer service we are implementing a new procedure, an Emergency Information Form, designed to:

  • Keep your child safe by pre-identifying authorized caregivers for pickup.
  • Provide current contact and medical information.
  • Inform us about special conditions.

In short we're working to keep your child safe and give you peace of mind!

Note: The emergency form is required for all summer camps.  Complete one form for each child, sign it and make multiple copies for every camp your child will attend.  (Only one form is needed for the JYSC or YSC programs.)

If you fill out your forms ahead of time for the entire summer, your work is done and you just need to hand in the pre-completed forms at the start of each week.

If you forget your Emergency Information form on the first day of camp, City staff will have extras for you to complete on-site.

Reminders

  • Completing forms ahead of time will save you precious minutes on the first day of camp each week and let you get going with your busy day.
  • Authorized persons for pickups will need a PHOTO ID.
  • If you are allowing your child to depart on his or her own, check off the appropriate permission box on the form.
  • Please pick your child up on time to avoid incurring late pick-up fees.
