Retiree Medical Reimbursement

Retirees who are eligible for the Retiree Medical Reimbursement program must provide proof of their monthly medical premium cost to the City of Fremont's Human Resources Department annually. It is also the retiree's responsibility to immediately notify the Human Resources Department regarding any changes to medical insurance premiums, direct deposit account, or contact information (email, mailing address, phone number).

Retirees should review the Annual Retiree Notice, linked below, to understand what actions are needed to satisfy the annual proof of coverage requirement. Retirees will also receive a copy of the notice by email or USPS mail prior to the new year

If you have any questions, please email us or call (510) 494-4660.
