Aide/Behavioral Specialist Form: Does your child have an aide or specialist that will attend with them at their registered camp/class? Submit this form. Aides/Specialists must be background checked and TB tested. Please submit this form at least 7 calendar days before the start of the activity.
Refund/Transfer Request Form: Submit your request for a refund or transfer through this form.
