Request for a Fire Incident Report
Please submit a Public Records Request through the City's Public Records website, Next Request.
In your request, clearly identify the incident:
- Date
- Approximate time
- Address/location
Request for Hazardous Materials & Fire Prevention Files
Please submit a Public Records Request through the City's Public Records website, Next Request.
Categories of files include:
- CUPA (Certified Unified Program Agencies)
- Underground storage tanks
- Fire Code Violations, Fire Inspections, etc.
In your request, clearly identify:
- Type of record(s)
- Timeline of record(s)
- Address
Request for Patient Care Record
These records are protected by HIPAA (Health Insurance Portability and Accountability Act) and cannot be requested through the City's Public Records website.
Request Process
If the requestor is the person/patient involved in the incident:
- Complete the Authorization for Release of Patient Care Records form
- Email it to Fremont Fire Department, fremontfire@fremont.gov
- If the requestor is the person/patient involved in the incident no supporting documents are needed
- You will be notified by email if the record(s) have been identified and processed for release or if we are unable to locate the record
- The records will be released as noted in the Authorization form
If the requestor is a third party (not the person/patient involved in the incident):
- Complete the Authorization for Release of Patient Care Records form
- Email it to Fremont Fire Department, fremontfire@fremont.gov
- Provide HIPAA compliant supporting documentation (this allows the Fire Department to legally release the Patient Care Report to a third party)
- You will be notified by email if the record(s) have been identified and processed for release or if we are unable to locate the record
- The records will be released as noted in the Authorization form.
Or Issue a Subpoena
- Issue a Subpoena Request to the Fire Department via the City Clerk's office
- Subpoenas can be hand-delivered or sent via postal mail
- If you are hand-delivering your subpoena, email the City Clerk's Office to schedule a delivery appointment

- The records will be released as noted in the Subpoena Request
Subpoena Request for Personnel
If you need to subpoena a member of the Fire Department:
- Issue a Subpoena Request to the Fire Department via the City Clerk's office.
- Subpoenas can be hand delivered or sent by postal mail
- If you are hand-delivering your subpoena, email the City Clerk's Office to schedule a delivery appointment
