Services

The City of Fremont participates in Alameda County’s Coordinated Entry System through the provision of the following Access Point services:

Housing Problem Solving
This is available to anyone at imminent risk of becoming homeless or those currently experiencing homelessness.

Crisis Assessment
This assessment is available to those who are literally homeless and is used to match eligible households to available shelter openings such as: emergency shelter, interim housing, or the City's Safe Parking Program. 

Housing Needs Assessment
This assessment is for the most vulnerable unhoused residents who are chronically homeless and have a mental health or physical disability so that they may be matched to available permanent supportive housing opportunities.
